Home Health Aide
Assists in home with care-related activities of daily living, both short-term and long-term.
Private Duty Nursing
Provides up to 23 hours of intensive care nursing to children and adults living at home requiring complex medical attention.
Clinical Care Management
Provides close oversight of Private Duty cases care, nurses, treatment, and medication.
Primary Care Management
Clinical oversight of intermittent cases ensuring optimal health, oversight of nurses, medication, and treatment.
Intermittent Nursing
Provides nursing services to post-acute patients in their homes, medication management with specialties in pediatrics and behavioral health.
Social Services
Provides assistance with access to community resources, financial applications for assistance and psychological support in the home.
Physical Therapy/Occupational Therapy/Speech Therapy
Service based on clinical deficits needed to achieve highest level of function to prevent or slow disease process.
Our Process
Nursing visits will consist of a complete physical assessment and evaluation of the condition. The skilled nurse will also instruct the patient and family/caregiver in disease process and management, medications and other pertinent topics. With case management, usually nursing visits will be frequent at first. Then as acute medical problems are resolved and home instruction completed, visit frequencies will decrease and discharge planning and teaching will begin. In some cases, the skilled nurse will make an initial visit only for admission assessment and evaluation.
Home Health Aides (HHA’s) are assigned by the skilled nurse and can provide personal hands on care for the patient as appropriate and mandated by the state regulations for HHA’s.
